Cementless Total Hip Arthroplasty in Patients With Seroid-Induced Osteonecrosis of the Femoral Head: Minimum Six-Year Follow Up Study

Abstract

|
Purpose : We analyzed the results of cementless total hip arthroplasty performed on patients with steroid induced osteonecrosis of the femoral head.
Materials and Methods : We analyzed 44 cementless, primary total hip arthroplasties performed on 32 patients, all of whom were diagnosed with steroid induced osteonecrosis of the femoral head. The underlying conditions necessitating steroid treatment were mainly renal transplantation and systemic lupus erythematosus. All had Ficat stage ¥²or ¥³ osteonecrosis of the femoral head. The mean follow-up period was 8.2 years (6~11), the mean age at operation 41 years (25~59). Patients were assessed by the clinical and radiographic results.
Results : The mean Harris hip score increased from 60 points preoperatively to 94 points at last follow-up. On radiologic evaluation, 44 stems met the criteria for bone ingrowth. none of the acetabular components showed evidence of measurable migration of loosening. Seven hips were revised for excessive wear of the polyethylene liner and progressive acetabular osteolysis.
Conclusion : The study demonstrates encouraging clinical and radiological results for cementless total hip arthroplasty in steroid-induced osteonecrosis. However, polyethylene wear and osteolysis remain serious concerns.
